When drafting agreements with healthcare professionals, use Template 5 or Template 6.

If it is the intention of the County to provide medical malpractice coverage for the contracted physician then please follow the instructions below and use Template 16. 

A non-standard indemnity agreement is required. Contact Risk Management before using this Template. Contact Risk Management if the Contractor is a corporation.

Physicians, Psychiatrists – County provides malpractice insurance – Sole proprietors
