Teen Parent Connections (TPC)
Teen Parent Connections (TPC) is a trauma-informed home visiting program for any pregnant or parenting teen ager 19 years or younger. Our social workers provide long-term comprehensive case management services and referrals within a local network of services and resources. TPC staff provide services to over 160 teenagers every year to maximize their educational potential and positive parenting. TPC has been shown to reduce the incidence of poor pregnancy outcomes including decreasing preterm births. Teens who work with TPC are more likely to delay subsequent pregnancies, have increased access to health care and are more likely to graduate.
