Health Services Update: Health Services Participates in National Dialogue on Creating an Optimal Health System

What should health care in America look like? Sonoma County was among 11 communities from 11 states that gathered in Chicago to address this question at an invitation-only roundtable convened by ReThink Health, an initiative dedicated to identifying innovative approaches to bridge health care delivery and community health initiatives.
ReThink Health invited groups known for creative health improvement models and took interest in Sonoma County because of our successful Health Action framework. The roundtable focused on core elements of stewardship, strategy and financing. The convening gave health care innovators a chance to explore opportunities for collaboration, share their innovative practices, align strategies, and expand upon existing initiatives to transform health care across regions.
Health Program Manager Jen Lewis represented Health Services and was joined by Karin Demarest, Vice President for Programs at the Community Foundation Sonoma County. They focused on Health Action’s efforts to address health system improvement, educational attainment, and economic wellness, and highlighted A Portrait of Sonoma County as a core component of our strategy to address health equity.
