RFP for Social Rehabilitation–Transitional Residential Program

The County of Sonoma Department of Health Services (Department), Behavioral Health Division (BHD) Adult Residential Services is pleased to invite you to respond to a Request for Proposals (RFP) for the Social Rehabilitation–Transitional Residential Program. This RFP outlines the information necessary to understand the competitive selection process and the required documentation necessary for the submission of proposals. All interested proposers must meet the requirements specified in this RFP. Proposers must have qualified staff on hand for this project and be ready to begin work on the date this project is initiated.
The Department of Health Services is seeking a Community Based Organization (CBO) toprovide a 9 (nine) bed Social Rehabilitation–Transitional Residential Program in Sonoma County to adults ages 18 years or older, who are ambulatory, and who meet criteria for Severe and Persistent Mental Illness (SPMI). Clients referred to this program would demonstrate some involvement in their recovery, but would also present with multiple mental health and life domain functional impairments, and identified risk factors for re-hospitalization or long-term care placement. The purpose of the program is to increase independent living skills, connect clients to natural supports, and divert clients from more restrictive long-term psychiatric settings.

The living wage rate required under the County’s Living Wage Ordinance (LWO) has increased. Effective January 1, 2022, the living wage rate required under the LWO shall be $16.75 per hour, an increase over the previous $15.00 per hour. The adjustment was adopted by the County’s Board of Supervisors on December 14, 2021, in accordance with Sonoma Municipal Code section 2-377(d). All covered employees must be paid at least the increased living wage rate.
